In this article, you will learn how to create an inspection form.
Please note: customers subscribed solely to "Hours of Service (New)" and not subscribed to "Inspect" will have a limited form builder feature set.

Navigate to Form Builder
To view Form Builder, follow these steps:
- In the left-hand navigation, select the "Inspect” service module.
- In the top navigation, select "Form Builder"
Form Builder Navigation Overview
Within Form Builder, you can
- View or search by form names (1)
- View the number of components (2)
- View or search by whom the inspection form was created by (3)
- View the number of assigned assets (4)
- View or search by the form's last inspection date (5)
- Create a new inspection form (6)
- Duplicate an inspection form (7)
- This allows you to create a copy of the inspection form and revise the form as desired
- Delete the form (8)
- Please note: standard IntelliShift inspection forms cannot be deleted
- View and edit the form (9)
- Please note: standard IntelliShift inspection forms cannot be edited. You must first make a copy of the form.
Create an Inspection Form
Use the following steps to create a new inspection form:

To create a new form, follow these steps:
- In the left-hand navigation, select the "Inspect” service module.
- In the top navigation, select "Form Builder"
- On the upper right-hand side of the screen, select the "Add Form" icon.
While building a form, you can
- Name the form (1)
- Add a form description (2)
- Add components (3)
- Components contain the check points operators are responsible for during an inspection. To learn more about Inspect form component options, see the article "Inspection Templates."
- Copy and duplicate the form (4)
- Delete the form (5)
- Add alert recipients for failed components or check point items (6)
- When a component or check point is marked as failed by an operator, the alert recipients will receive an email upon operator submission.
- Assign assets (7)
- The assets assigned will have access to the form.
- Set rules for automation (8)
- Rule automation includes Compliance Template, Create Issues on Checkpoints, Inspection Required, Operator Assignment, Block Inspections for ongoing issues. To learn more about Inspect form rules, see the article "Inspection Templates."
